Surgical excision is, however, associated with a considerable burden of recurrences, with reported recurrence rates after surgery. Recurrence of an ovarian endometrioma is, therefore, a common occurrence, and the clinician is often faced with the decision of whether undertaking a second surgical procedure or managing the recurrence expectantly or with medical therapy.

Many ovarian cysts will go away over time or do not change. However, watchful waiting is controversial and some physicians believe benign cysts should always be removed. If the cyst doesn’t go away after several menstrual periods, or if it gets larger, your doctor may want you to have an operation. Your doctor may want you to have surgery if your cyst doesn’t look like a functional cyst on the sonogram. Many ovarian cysts in women of childbearing age require surgery.
