I am not a medical person. The protruding, I think, is the result of the body being starved, the stomach is full of air. Anorexic individuals will look the same way. There are other things which occur, e.g., listlessness and loss of muscle mass. The body is breaking down, but trying to save itself from imminent danger. The belly’s distension is an indication that death is close at hand. People with anorexia have a mental condition where they don’t see themselves in reality. They are confused and delusional.
